Who is Eligible?

BurchCreek HomeCare

An individual who is homebound, has had a recent illness or injury, and requires care on an intermittent basis.  You must qualify for a skilled need such as physical therapy or nursing care.

BurchCreek Hospice Care

Any individual who has been certified by a physician to be terminally ill with a life expectancy of 6 (six) months or less and choose palliative (comfort) care rather than life prolonging curative care.
